The Community Based Therapist provides support and guidance to the caregivers of the youth and case management services to enhance the success of the youth and their dynamics within the home and Job Responsibilities:Complete required assessments and establish medical necessity for all youth in programDevelops and implements treatment plans based on assessment and evaluation and provides input into youth treatment goals for other team psychosocial education and therapy to youth and families in a home-based crisis management and intervention services both during business hours and thru the after- hours on-call cell and update case records electronically and in hard case Qualifications:Licensure (LCSW LMSW LMHC LMFT) or Limited Permit and Masters in Social Work or related clinical degree and one (1) year post-graduate experience working with children and adolescents. Must have a valid New York State drivers license and access to reliable Required Skills and Abilities:Is sensitive to cultural differences and able to incorporate that understanding into service provision. Strong organizational and interpersonal skills are needed. Must be able to work independently and with flexibility in days and hours of work. Excellent verbal and written communication skills and computer literacy are required.
