CNA PRN Hospice- Houston

Interim HealthCare one of the nations leading providers of home care is now seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our hospice care team. As a Hospice CNA you will play a crucial role in delivering high-quality personalized end-of-life care to patients in their homesensuring their comfort dignity and overall well-being during this sensitive phase of life.

Responsibilities:

Patient Care:

Provide hands-on assistance with activities of daily living (ADLs) including bathing dressinggrooming and monitoring vital signs.

Document any changes in the patients condition and report concerns to the registered nurseor hospice team.

Medication Administration:

Assist in administering medications as per prescribed treatment plans and report anyadverse reactions or side effects.

Maintain accurate records of medication administration.

Comfort Care:

Maintain a clean safe and comfortable environment making necessary adjustments toensure patient well-being.

Implement appropriate pain management techniques as outlined in the patients care plan.

Documentation:

Maintain detailed patient records documenting care provided observations and anychanges in the patients condition.

Ensure compliance with all documentation and reporting requirements as per hospiceregulations.

Collaboration:

Work collaboratively with the interdisciplinary team including nurses social workerschaplains and volunteers to deliver holistic care to patients.

Participate actively in team meetings and contribute to care planning discussions.

Qualifications:

Licensed CNA in the State of Texas - required

1 years experience - preferred

Experience with Hospice or Home Health - preferred

Effective written and verbal communication

Ability to travel - must have reliable transportation

Clean background check and drug screen
