We are seeking a professional customer-focused Home Care Intake & Client Services Coordinator to join a growing home care organization serving Connecticut.

This position serves as a primary point of contact for individuals seeking home care services. You will answer inbound calls gather client information explain available services coordinate assessments and connect prospective clients with the appropriate care team.

The position is primarily remote; however the Coordinator will occasionally assist with field visits and client-related needs in Litchfield County. For this reason candidates must live in or near Litchfield County and have reliable transportation.

The ideal candidate is a CNA with previous home care experience who understands the needs of clients receiving services in their homes and is comfortable balancing telephone-based intake and client service responsibilities with occasional field support.

Answer inbound calls from prospective clients family members referral sources and healthcare consumers

Gather and accurately document client information

Explain available home care services and next steps

Schedule assessments consultations and follow-up appointments

Coordinate new client intake with the appropriate operational and clinical teams

Provide compassionate and professional customer service throughout the intake process

Maintain ongoing communication with clients families caregivers and office staff

Assist with client service needs and follow-up

Conduct occasional field visits within Litchfield County as needed

Help identify changes in client needs and communicate concerns to the appropriate team members

Assist with ensuring a smooth transition from initial inquiry through the start of care

Communicate effectively by phone email text and virtual platforms

Respond to inquiries promptly and professionally

Follow up with prospective clients and referral sources

Help clients and families understand available services and the next steps in the process

Maintain accurate records within intake scheduling and client management systems

Document client interactions and communications

Track inquiries referrals assessments and follow-up activities

Ensure client information is accurate and complete

Address client and family questions and concerns

Escalate clinical or complex situations appropriately

Collaborate with caregivers office staff and clinical teams to ensure a positive client experience

Active CNA certification required

Home care experience required

Home care intake client services scheduling or care coordination experience strongly preferred

Must live in or near Litchfield County Connecticut

Ability to make occasional field visits throughout Litchfield County

Reliable transportation and valid drivers license required

Excellent verbal communication and active listening skills

Strong organizational and time-management skills

Ability to work independently in a remote environment

Comfortable learning and working with healthcare software and technology

High-speed internet connection and dedicated home workspace

Home care intake or scheduling experience

Healthcare customer service experience

Client services or care coordination experience

Medical scheduling or patient access experience

Intake or referral coordination experience

Medicaid Medicare or managed care experience

Experience communicating with clients families caregivers and referral sources

Bilingual English/Spanish is a plus

$19.00 per hour

Primarily remote work environment
