With 50 locations and 100 programs in NYCs most under-served communities our 1000 results-oriented professionals make a difference for more than 40000 children and their families each Child Center of NY is seeking a dedicated and compassionate Clinical Support Staff member to join our Youth Assertive Community Treatment (ACT) team. The Youth ACT program provides intensive community-based services to children and adolescents with serious emotional disturbance and their Clinical Support Staff is responsible for providing direct support services to youth and families assisting with case management skill-building and community linkage. Working under the guidance of licensed clinicians this role helps clients engage in treatment access needed resources and achieve their goals in the home and community FunctionsProvide direct support services to children adolescents and families in home and community clients with skill development in areas such as daily living social interaction coping strategies and youth and families in accessing and navigating community resources including educational vocational medical and social transportation assistance for clients and families as needed to facilitate access to services and with the interdisciplinary ACT team to coordinate care and reinforce treatment in daily team meetings and case reviews to monitor client progress and identify next in crisis response and provide de-escalation support in collaboration with licensed team accurate and timely documentation of services provided in the agencys Electronic Health Record (EHR).Engage families in treatment planning encouraging participation and shared in outreach and engagement activities to promote client involvement in other related duties as QualificationsBachelors degree required in a related field (social work psychology human services) strongly preferredExperience working with children adolescents and families in behavioral health youth development or social communication engagement and relationship-building to work effectively as part of an interdisciplinary to provide services in the community including evenings and weekends as to use public transportation for client visits when Opportunity EmployerThe Child Center of NY is an Equal Opportunity Employer.
