Job Summary

The Social Worker is responsible for performing social work assessments and interventions as needed for inpatients outpatients and emergency department patients. They are dedicated to patient and family-centered care that values personal self-determination skilled at managing multiple priorities at once and engaging in creative compassionate and ethical problem-solving. As a member of a multidisciplinary health team they perform comprehensive assessments and develop treatment plans that integrate the medical social and resource issues that impact individual patients families and at-risk populations. Social Workers are responsible for working collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.

Essential Functions:

-Provides psychosocial assessments of patients and families with social emotional interpersonal and/or environmental issues.
-Formulates biopsychosocial assessment disposition and treatment plans.
-Collaborate with other professionals to evaluate patients medical or physical condition and to assess client needs as part of the interdisciplinary team.
-Monitor evaluate and record client progress according to measurable goals described in the treatment and care plan.

Qualifications:

• Masters Degree Mental Health & Behavioral Medicine or Masters Degree Social Work required

• Licensed Independent Clinical Social Worker (LICSW) Licensed Mental Health Counselor (LMHC) or Licensed Clinical Social Worker (LCSW) required

• 2 years of clinical experience in a medical setting required

• Class D Passenger Vehicle Drivers License required

Additional Knowledge Skills & Abilities:

• Exceptional clinical judgment and application of professional values and ethics. Integration of prior clinical experience into current practice.

• Exceptional ability to formulate assessment of risks crisis intervention psychosocial assessments treatment plans and the documentation of both in a timely manner.

• Exceptional clinical judgment and innovation in advocating for clients while maintaining professional boundaries.

• Working knowledge of internal and external resources and refers appropriately.

• Provides a range of evidence-based interventions.

• Ability to advocate coordinate and continuously communicate with treatment plans with patients family and the interdisciplinary team.

• Incorporates social sensitivity.

• If appropriate utilizes supervision and consultation regularly and appropriately

• Actively seeks assistance with complex case s and situations in a timely manner.

• Engages in quality improvement projects uses data to measure progress.

• Facilitate skill-based groups.

• Knowledge understanding and ability to negotiate and work in a complex organization.

• Effectively communicates findings with a broader audience.

Schedule and Working Model:

• Per Diem. Must be available to work a minimum of 16 hours per week preferably up to 40 hours per week. Hours worked will fall between 8:30am-5pm Monday through Friday.

• Travel within the Boston MA area required (Back Bay Beacon Hill Chinatown Dorchester Kenmore Jamaica Plain Mattapan Mission Hill North End Readville Roslindale Roxbury South End South Boston Quincy Randolph)

• Days Per Diem shifts available

• M- F 8:30-5 . No on call no weekends

• Remote workdays require stable secure quiet compliant work area

This is a hybrid role in that phone calls follow up calls meetings case discussions and huddles can be taken from home remotely in a quiet office with MGB issues equipment. The schedule seeing patients in the community will vary fromday to day and week to week. Social Worker must be flexible and able to see patents in the community as needed on all the days they commit to work.
