Job Summary

Responsible for assigned documentation review of hospice service delivery to ensure state and federal quality and compliance requirements are met by all care teams including Patient Access Patient Services and Hudson Valley Hospice House. Assists with education and follow-up to clinical staff to promote high-quality care. This includes but is not limited to HOPE admission the HOPE Update Visits (HUV) 1 and 2 the HOPE discharge and the Symptom Follow Up (SFV) visits. Required to establish and maintain positive relationships with hospice staff patients nursing facilities vendors and referral sources


What you will do:

  • Review documentation related to all aspects of pre-admission activities evaluation initial and comprehensive assessment and other regulatory determined timepoints (e.g. HOPE) thoroughly in the EMR system ensuring all required elements are captured accurately for timely quality data submission. This may include completion of quality data sets based on EMR review.
  • Complete compliance and quality audits assigned.
  • Follow up with clinicians and leaders as indicated by documentation reviews including instruction and guidance of clinicians to promote more effective performance and delivery of quality care.
  • Follow federal state and agency guidelines to assist the hospice team in supporting clinical eligibility for hospice services in documentation.
  • Assist clinicians in establishing and updating the POC including but not limited to medication review medication entry in EMR and/or e-scribe system and POC goals and interventions.
  • Ensure timely scheduling and accurate completion of HOPE HUV and SFU visits to ensure compliance with state and federal regulations.
  • Track any trends while reviewing quality metrics including but not limited to the HOPE tool report findings to VP of Mission Quality and COO.
  • Send and track return of regulatory required documents as needed (eg. election of benefits).
  • Assist with review and preparation for external audits (eg. ADR TPE CERT RAC etc)

  • Attention to detail.
  • Bilingual is a plus!

Work Environment: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

  • Working within assigned settings including but not limited to Hudson Valley Hospice offices hospitals patient/family homes nursing homes and assisted living facilities (if necessary)
  • Ability to carry up to twenty-five pounds of supplies and/or materials.
  • Capability to work a flexible schedule to meet the needs of the patients including occasional weekends and holidays.
  • Ability to provide transportation for self and access public buildings to provide community education.
  • Ability to drive and work in winter weather including snow.

Education & Experience:

  • Graduate from accredited nursing program
  • Valid RN License in New York State.
  • Valid drivers license in New York State.
  • Experience working in a fast-paced environment preferred.
  • A minimum of two years experience in home care or hospice is preferred.
  • One year of healthcare quality and/or compliance work preferred.
  • Experience and comfort in caring for terminally ill patients.
  • Experience and comfort in use of EMRs and other electronic platforms
  • A basic understanding and commitment to the hospice philosophy.

Full Time

  • Monday - Friday
  • 8:30am-4:30pm

Pay: $/yr
