Were hiring an Ambulatory Clinical Pharmacy Specialist who will play a hands-on role in elevating patient care at a nationally recognized Academic Medical this dynamic patientcentered position youll build meaningful relationships with patients and providers while delivering highquality medication management. Youll work remotely collaborating directly with care teams and practicing under a Collaborative Practice Agreement (CPA) to conduct virtual patient assessments order and monitor evidence-based treatment plans optimize therapy and resolve medication-related problems.
Location: Hybrid role in Centralia/Olympia/Lacey area
- Potential Locations: Providence Centralia Hospital Providence St. Peter Hospital
Salary:An employee in this position can expect a salary rate between $127000 and $142000 (Hourly Equivalent: Approximately $61.06 - $68.27 per hour based on a 40-hour workweek.) plus bonus pursuant to the terms of an any bonus plan if applicable. The actual salary will depend on experience geographic location and other factors permitted by law.

What Youll Do:
- Provide comprehensive medication management including reconciliation dose optimization adherence support and druglevel assessments aligned with evidencebased standards.
- Partner with physicians specialists and interdisciplinary teams to order individualized care plans under a CPA and promote seamless medication coordination.
- Monitor therapy effectiveness and safety identify optimization and intervene as appropriate.
- Support qualityimprovement and REMS initiatives.
- Provide recommendations to operations by supporting teammates as needed with complex prior authorizations improving workflows educating patients and maintaining ongoing clinical development.
What You Need:
- Hold a Doctor of Pharmacy (PharmD)
- Must be a licensed pharmacist in Washington
- Candidate must meet one of the following qualifications:
- Completion of a PGY2 Ambulatory Care Residency
- Completion of a PGY1 Residency and at least 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA)
- At least 3 years of ambulatory practice with disease state management
- Strong communication organization and tech skills

Solid computer skills are required including proficiency in Microsoft Office tools (Outlook Teams Word Excel and PowerPoint) for communication documentation and data management as well as navigating the internet and web-based platforms.
