Job Description:

The Client shall be considered to be competent when he/she consistently demonstrates the ability to transfer scientific knowledge from social biological and physical sciences in applying the nursing process as follows:
Provides care that is consistent with the interdisciplinary plan of care and grounded in the principles of Relationship Based Care.
Assess patient physical and psychosocial condition on an ongoing basis and formulates a nursing diagnosis through observation of the clients physical condition and behavior and through interpretation of information obtained from the client and others including the health team.
Formulates a care plan in collaboration with the client which ensures that direct and indirect nursing care services provide for the clients safety comfort hygiene and protection and for disease prevention and restorative measures.
Performs skills essential to the kind of nursing action to be taken explains the health treatment to the client and family and teaches the client and family how to care for the clients health needs.
Evaluates the effectiveness of the care plan through observation of the clients physical condition and behavior signs and symptoms of illness and reactions to treatment and through communication with the client and the health team members and modifies the plan as needed.
Demonstrates a current working knowledge and safe use of equipment instrumentation and medications.
Delegates tasks to care team members based on their legal scopes of practice and on the preparation and capability needed in the tasks to be delegated and effectively supervises nursing care being given by the care team.
Acts as the clients advocate as circumstances require by initiating action to improve health care or to change decision or activities
which are against the interests or wishes of the client and by giving the client the opportunity to make informed decisions about health care before it is provided.
Provides culturally competent age appropriate ethical care within legal standards of practice.
Acts as a liaison in coordinating patient care activities with nurses physician allied health professionals and ancillary care team members.

Performs other duties as assigned. Proficient written verbal and electronic communication skills.
Performs all physical duties of the job.
Requires lifting bending pushing pulling sitting standing walking kneeling twisting reaching and fine motor skills.
Completion of annual organizational requirements for employment.
