Clinical LTSS Service Care Manager

Job Title: LTSS Service Care Manager
Assignment Duration: 6-month contract with potential for extension or conversion
Schedule: Monday Friday 8:00 AM 5:00 PM

Work Type: Delaware: New Castle County
Travel Requirement: Yes - member home and site visits required

Key Responsibilities

Able to assist members with immediate needs. Able to support LTSS team on maintaining members compliant with level of care redeterminations.

Assists with developing ongoing long-term care plans/service plans and works to identify providers specialist and/or community resources needed for long-term care .

Coordinates as appropriate between the member and/or family/caregivers and the care provider team to ensure identified services are accessible to members.

May perform home and/or other site visits to assess members needs and collaborate with healthcare providers and partners.

Required Qualifications

Bachelors degree and 2 4 years of related experience.

Delaware/Compact RN License

Drivers license

Computer literate

Experience on evaluating older adults/disabled level of functioning

Experience with medication reconciliation
