Clinical Liaison Hospice

Location:La Jolla CA and surrounding North SD County areas

Position Summary

Salus Hospice is seeking a dynamic and clinically knowledgeableClinical Liaisonto serve as the primary connection between hospitals physicians and our hospice care services.

This role is critical in ensuring seamless patient transitions from acute care settings to home-based services while maintaining strong relationships with referral sources and delivering exceptional patient-centered care.

Key ResponsibilitiesPatient Transition Management

• Coordinate and facilitate smooth transitions from hospital or facility to home care
• Reduce re-hospitalizations through proactive discharge planning
• Ensure continuity and accuracy of patient care plans

Clinical Coordination

• Assess patient eligibility for home health palliative or hospice services
• Collaborate with physicians and care teams to determine appropriate levels of care
• Support development and execution of individualized care plans

Strategic Outreach & Relationship Building

• Develop and maintain strong referral relationships with hospitals physicians and healthcare providers throughout San Diego
• Serve as a trusted clinical resource for partners and families
• Represent Salus in the community to promote service awareness

Regulatory & Compliance Expertise

• Maintain knowledge of Medicare insurance guidelines and hospice eligibility criteria
• Ensure all admissions and referrals meet regulatory standards

Qualifications

Required:

• ActiveLVN or RN licensein the state of California
• Minimum12 years of experiencein hospice palliative care home health or healthcare marketing/liaison role
• Strong understanding ofMedicare and insurance guidelines
• Excellent communication and relationship-building skills
• Ability to work independently in a fast-paced field-based environment

Preferred:

• Prior experience as aClinical Liaisonor in business development within healthcare
• Knowledge ofSan Diego healthcare systems and referral networks
• Experience in interdisciplinary care coordination

Work Environment & Schedule

• Field-based role coveringSan Diego County
• Full-time with flexible scheduling based on referral and facility needs
