Clinical Liaison & Patient Acquisition Specialist
Job Summary
This is a remote position.
The Offshore Clinical Liaison & Patient Acquisition Specialist is responsible for helping the agency generate capture respond to and coordinate new patient opportunities across both B2B referral sources and direct patient/family inquiries.
This is not a traditional virtual assistant position.
The specialist operates as the agencys virtual Patient Acquisition Command Center combining:
AI-powered prospecting referral-source outreach electronic referral monitoring 15-minute speed-to-lead intake coordination CRM management.
The primary objective is:
Generate more qualified referrals respond to every opportunity quickly and help convert appropriate referrals and inquiries into assessments and admissions.
The specialist works closely with U.S.-based clinical and operations personnel. The position supports referral development and intake coordination but does not independently make clinical decisions requiring licensed professional judgment.
Build and maintain relationships with potential referral sources including:
- Hospitals and discharge planners
- Skilled nursing facilities
- Rehabilitation facilities
- Physician practices
- Outpatient clinics
- Senior living communities
- Assisted living communities
- Elder-law professionals
- Case managers
- Social workers
- Community organizations
- Other healthcare referral partners
Research organizations and appropriate decision-makers maintain referral-source records conduct approved outreach follow up consistently identify opportunities and route qualified conversations to agency leadership or field clinical liaisons.
The goal is not simply to send emails.
The goal is to develop referral relationships that produce patients.
Use AI and approved sales technology to build and operate the agencys referral-development engine.
Responsibilities include:
- Research target referral organizations.
- Identify appropriate decision-makers.
- Build and clean prospect lists.
- Enrich contact information.
- Segment referral sources.
- Use AI to research and personalize outreach.
- Launch approved cold-email campaigns.
- Support compliant SMS outreach where appropriate.
- Manage follow-up sequences.
- Monitor responses.
- Categorize positive negative neutral and follow-up replies.
- Route warm opportunities immediately.
- Maintain accurate outreach records in the CRM.

Required Skills:
Assist patients with activities of daily living (ADLs) including bathing dressing grooming toileting feeding and mobility support using assistive devices such as Hoyer lifts and transfer equipment Measure monitor and accurately record vital signs (blood pressure pulse temperature respiration) in electronic health record (EHR) systems such as Epic Cerner Meditech or eClinicalWorks Support licensed nursing staff with patient observation rounding and routine clinical tasks while adhering to HIPAA and facility protocols Provide compassionate care for patients with dementia Alzheimers disease behavioral health needs or post-acute rehabilitation requirements Collaborate with nurses physicians and interdisciplinary care teams to deliver coordinated patient-centered care Maintain infection control standards including aseptic techniques during wound care catheter care and isolation procedures Assist with patient transfers ambulation and positioning while ensuring safety and proper body mechanics Support rehabilitation and recovery programs by assisting patients with exercises mobility routines and daily activities Deliver respectful and compassionate care to diverse patient populations primarily adult and geriatric patients Document all patient care activities thoroughly and accurately within facility-approved medical documentation systems Experience & Qualifications Current and valid New York State CNA certification Previous experience as a CNA in hospitals long-term care facilities rehabilitation centers assisted living communities or similar healthcare settings preferred Experience in acute care medical-surgical units or high-acuity environments is a plus Familiarity with EMR/EHR systems such as Epic Cerner Meditech eClinicalWorks or similar platforms Knowledge of infection control practices patient safety standards and basic medical terminology Ability to work effectively within multidisciplinary healthcare teams Strong communication skills reliability and attention to detail BLS certification preferred
