Position SummaryThe Case Manager plays a vital role in delivering client-centered services that support patients across the continuum of care. This position partners with patients and their families to coordinate healthcare and connect them with appropriate home- and community-based services.
You will ensure timely access to medically appropriate care while promoting continuity quality outcomes and a positive patient experience.

- Conduct comprehensive patient assessments and develop individualized care plans
- Coordinate care across providers and service settings
- Advocate for patients and their families to ensure access to necessary resources
- Provide education to patients caregivers and support systems
- Monitor patient progress and adjust care plans as needed
- Maintain accurate timely and compliant documentation
- Deliver age-appropriate care based on developmental needs

- Active unrestricted Registered Nurse (RN) license in the State of Florida (must be maintained)
- 3 years of clinical experience
- 1 year of case management experience preferred
- Strong computer proficiency including Excel
- Experience with Electronic Medical Records (EMR) systems (Athena experience a plus)
- Excellent communication organization and problem-solving skills
