What Youll Do:
As a Clinic RN you will be responsible for providing both direct and indirect nursing care to PACE this role you will manage complex clinical situations exercise sound clinical judgment and support high-quality participant-centered care as part of the interdisciplinary team.
Key Responsibilities
- Provides skilled nursing services in accordance with the RN scope of practice including but not limited to wound care catheter care IV therapy medication administration injections and specimen collection.
- Provides supportive hands-on skilled clinical nursing care to meet the day-to-day needs of PACE participants.
- Responds to acute concerns and changes in participant condition by completing focused nursing assessments.
- Obtaining appropriate vital signs and clinical information.
- Escalating concerns to the appropriate provider when indicated.
- Assist with semiannual nursing assessments as needed to support the Primary Nurse.
- Works closely with nurses medical assistants providers and other members of the interdisciplinary team to coordinate participant care.
- Completes accurate timely and thorough documentation of nursing assessments interventions treatments participant response and communication in the electronic medical record.

Skills Knowledge and Expertise
- Graduate of an accredited School of Nursing.
- Licensed Registered Nurse in the State of Michigan.
- Must have three (3) years experience as a practicing nurse in a health-related area.
- Must be posses a valid drivers license.
- Must be CPR certified.
- Minimum of one (1) year working with the frail and elderly.
