Pay: $65000/year performance bonus mileage reimbursement

What Youll Do

Prospect Calls & Follow-Up

• Answer and return calls from families asking about care
• Listen to their situation explain our services and schedule intake visits
• Follow up with prospective clients who havent started service yet
• Keep call notes and follow-ups up to date in our system

Client Intake

• Visit new clients and families in their homes to learn about care needs routines and preferences
• Walk through home safety and help us match the right caregiver
• Document each intake visit
• Stay a friendly point of contact for families as care begins

Community Outreach

• Build and maintain relationships with local referral sources such as senior living communities discharge planners and doctors offices
• Represent Company Name at community events
• Track outreach activity and referral results

What Were Looking For

• 5 years in the healthcare industry preferred (home care home health hospice senior living or a medical office)
• Experience with intake admissions sales or referral development strongly preferred
• Existing Houston-area referral relationships are a big plus
• Reliable vehicle valid drivers license and auto insurance
• A warm confident phone manner comfortable talking with families and professionals
• Organized and good at follow-through
• Flexibility for occasional evening and weekend client intakes

Schedule
Full-time primarily weekdays. Occasional evening and weekend client intakes may be needed since families often reach out when a loved one needs care right away.
