Client Care RN Part Time

Objective:

The Client Care RN is expected to perform a variety of duties. The Client Care RN is expected to work closely with Client Care Coordinator and Staffing Coordinators in order to provide the highest quality of service to clients.

Primary Responsibilities:

• Facilitate effective communication between Clients families and Caregivers by answering questions related to client care
• Perform assessments of personal care and companion/home helper Clients and create plan of care
• Maintain an up to date record of client care plans and client information
• Counsel CAREGivers when questions arise about care related to the personal care needs of our clientele
• Contact family members/emergency contacts with concerns when deemed appropriate
• Complete paperwork according to state guidelines
• Monitor mediate and log both client and CAREGiver activity and follow up
• Demonstrate open and effective communication with owner colleagues and CAREGivers
• Address health related concerns of companion level clients
• Report any changes in client condition to appropriate staff
• Visit client if there is a reason for concern
• Maintain regular attendance at the office to execute job responsibilities
• Assess clients when there is a change in condition hospitalization or change in level of care
• Conduct Quality Assurance visits with high level companion/home helper clients
• On-call responsibility includes being available for calls regarding client care emergencies care consultations and CAREGiver questions on designated weekends

Secondary Responsibilities:

• Teach Personal Care class
• Complete Supervisory Visits as needed
• Review maintain file and audit personal care notes
• Perform any and all other functions and responsibilities deemed necessary

Critical Numbers:

• Report assessments performed or to be performed
• Report any supervisory visits done or to be done
• Report any quality assurance visits done

Education/Experience Requirements:

• Minimum BS in Nursing
• Must have a valid and current Virginia registered nurse license with no disciplinary actions administered by the Board of Nursing
• One year of related business experience or an equivalent combination of education and work experience may be considered
• Must possess a valid drivers license

Knowledge Skills and Abilities:

• Must have an understanding of and uphold the policies and procedures established by Asilinge LLC (d.b.a. an independently owned and operated Home Instead Senior Care franchise)
• Must demonstrate excellent oral and written communication skills and the ability to listen effectively
• Must have the ability to work independently maintain confidentiality of information and meet deadlines
• Must have the ability to demonstrate effective interpersonal skills is essential as well as sound judgment and good decision-making
• Must demonstrate discretion integrity and fair-mindedness consistent with company standards practices policies and procedures
• Must have the ability to organize and prioritize daily monthly quarterly and yearly work
• Must have the ability to establish good working relationships with office colleagues the franchise owner clients and the community
• Must have the ability to present a professional appearance and demeanor
• Must have the ability to operate office equipment
• Must have computer skills and be proficient in Word Excel Outlook and ClearCare
• Must be able to work evenings or weekends as required
• Must have knowledge of the senior-care industry
• Must have the ability to sit at a desk and listen effectively for long periods of time on the telephone
• Must be patient and congenial on the telephone
• Must be able to perform duties in a professional office setting
