Client Care Manager

• Conduct non-clinical client assessments and establish service plans
• Perform supervisory and quality assurance visits
• Coordinate services for clients and caregivers
• Coach mentor and train caregivers
• Support client transitions from hospitals rehabilitation centers and assisted living communities
• Collaborate with discharge planners case managers social workers and facility staff
• Investigate concerns and resolve service issues
• Review documentation and identify opportunities for improvement
• Support infection prevention safety and compliance initiatives
• Assist with leadership and oversight of Care Coordination and Staffing teams
• Build and maintain strong referral source relationships

• RN LVN Social Worker or healthcare professional with comparable leadership and care coordination experience
• 35 years of experience in home care healthcare operations hospice home health case management caregiver supervision or senior services preferred
• Strong communication and relationship-building skills
• Excellent organizational and problem-solving abilities
• Proficiency with Microsoft Office and technology platforms
• Ability to work independently while managing multiple priorities
• Valid drivers license and reliable transportation
