Description of Work
This position provides technical assistance guidance and subject matter expertise to regional Rural Organizations Orchestrating Transformation for Sustainability (ROOTS) Hubs implementing chronic disease goals within the Rural Health Transformation Program. The role supports program planning and execution across cancer prevention and control early detection and screening and tobacco/nicotine use prevention and cessation. Responsibilities include analyzing chronic disease trends advising on evidencebased clinical and community strategies and strengthening provider engagement and multidisciplinary care models. The position also coordinates Hub data collection monitoring and interpretation related to cancer and tobacco outcomes.Key Duties Include:
-Assess regional Hub needs related to cancer prevention cancer screening improvement tobacco/nicotine cessation integration and chronic disease management.
-Guide Hubs in selecting and adapting evidencebased chronic disease interventions for rural and medically underserved populations.
-Provide ongoing technical assistance through structured calls trainings office hours and oneonone consultations focused on cancer control and tobacco cessation strategies.
-Review Hub workplans deliverables and implementation progress to provide actionable recommendations aligned with program standards.
-Support Hubs in implementing screening qualityimprovement activities and provider engagement strategies within clinical workflows.
-Assist Hubs in integrating evidencebased cessation services including Quitline referrals counseling approaches and pharmacotherapy protocols.
Coordinate the collection validation and analysis of cancer and tobaccorelated data to support planning monitoring and evaluation.
-Compile data and performance metrics for required reports to funders and stakeholders emphasizing screening uptake cessation activity and rural cancer burden.
-Maintain and disseminate cancer and tobaccorelated program resources guidance documents and tools to support Hub implementation.
-Facilitate peer networking and informationsharing opportunities focused on effective chronic disease cancer control and tobacco cessation strategies.

Salary Grade Range: $60881.00 - $106544.00
Recruitment Range: $60881.00 - $80593.00

Demonstrated experience applying public and community health principles to chronic disease cancer control or tobacco/nicotine cessation initiatives reflecting intermediate proficiency in program implementation.
Proven skill delivering training and technical assistance to community or regional stakeholders including facilitating structured consultations virtual office hours or capacitybuilding sessions.
Applied experience collecting validating and analyzing cancer or tobaccorelated data to inform planning performance monitoring and evaluation activities.
Demonstrated effectiveness communicating technical guidance to program stakeholders across diverse settings including clinical partners community organizations and multidisciplinary teams.
Documented experience leading or supporting initiatives aimed at reducing cancer burden or decreasing tobacco/nicotine use including implementing evidencebased interventions within rural or underserved populations.

Division of Public Healths Chronic Disease and Injury Section along with local health departments and other community partners works to reduce death and disabilities through education policy change and various services. Our goal is to help all North Carolinians develop healthy and safe communities and health systems to prevent and control chronic diseases and to eliminate health inequities.
NC Rural Health Transformation Program is a statewide program led by NCDHHS to transform rural health care delivery improve health outcomes and ensure sustainable access to care for nearly 3 million rural North Carolinians. The program is funded over five years by the Centers for Medicare & Medicaid Services (CMS).
The main goals of the program are to:
Catalyze innovative health care models for rural communities by changing the way providers work together to care for patients in rural NC.
Transform the rural care experience building community-based clinical behavioral and social supports close to home.
Create a sustainable rural health delivery system through underlying systems changes in rural workforce pipelines and care team models and rural provider financial models.
