Job Description:
- Conduct psychosocial assessments of patients and families
- Develop and implement individualized care plans
- Provide counseling crisis intervention and emotional support
- Coordinate community resources and support services
- Assist with Medicaid disability and other assistance program applications
- Participate in interdisciplinary care team meetings
- Maintain accurate and timely documentation
- Ensure compliance with Medicare Medicaid and agency guidelines
Requirement:
- Bachelors or Masters degree in Social Work (MSW preferred)
- Current state Social Worker license/certification
- 1 2 years of social work experience (Home Health Hospice or Healthcare preferred)
- Knowledge of community resources and healthcare services
- Valid drivers license and reliable transportation
- Strong communication counseling and documentation skills
- Home Health or Hospice experience
- MSW degree
- Knowledge of Medicare/Medicaid regulations
