Job Description:
- Conduct patient evaluations and reassessments
- Develop and implement individualized treatment plans
- Provide therapeutic exercises gait training balance training and mobility training
- Educate patients and caregivers on home exercise programs and safety
- Complete timely documentation and progress reports
- Collaborate with physicians and healthcare team members
- Ensure compliance with Medicare and agency guidelines
Requirement:
- Graduate of an accredited Physical Therapy program (DPT or equivalent)
- Current Physical Therapist license
- Current CPR certification
- Minimum 1 year of Physical Therapy experience (Home Health preferred)
- Valid drivers license and reliable transportation
- Strong communication and documentation skills
- Ability to work independently
