Job Summary
Duties:
- Send facility placement referrals
- Follow up with nursing home liaisons and interdisciplinary staff to secure appropriate placements
- Reply to requests for additional information
- Keep statistical and progress reports
- Attend ALOC meeting and provide updates
- Take steps to overcome barriers to placement
- Send out timely referrals follow on requested information and respond to social work staff regarding outcomes
Skills:
- Similar background 3 years of experience is required.
- Social services excellent verbal and written communication and computer literacy. - Required
- EPIC and/or careport.- preferred
Education:
- Baccalaureate Degree from an accredited college or university.
Schedule Notes:
- This is onsite role. Home visits are not needed.
