Case Worker

Duties:

• Send facility placement referrals
• Follow up with nursing home liaisons and interdisciplinary staff to secure appropriate placements
• Reply to requests for additional information
• Keep statistical and progress reports
• Attend ALOC meeting and provide updates
• Take steps to overcome barriers to placement
• Send out timely referrals follow on requested information and respond to social work staff regarding outcomes

Skills:

• Similar background 3 years of experience is required.
• Social services excellent verbal and written communication and computer literacy. - Required
• EPIC and/or careport.- preferred

Education:

• Baccalaureate Degree from an accredited college or university.

Schedule Notes:

• This is onsite role. Home visits are not needed.
• 9:00 AM-5:00 PM
