Job Summary
Case Worker needed for full-time temporary role in Montgomery County.
The Case Worker coordinates inpatient and post-acute services including assessments treatment planning discharge planning and aftercare coordination. This role collaborates with patients families physicians and community providers to identify needs secure resources and develop effective discharge plans from admission through transition to the next level of care. Responsibilities include documenting all discharge-related activities in the TIER system facilitating referrals and providing guidance and support to help patients and families navigate the impact of illness and plan for successful recovery.
