Job Summary

Navitas Healthcare LLC is seeking an experienced Case Worker to provide social services and care coordination support for individuals and families requiring assistance with social medical and community-based resources. The Case Worker will assess client needs develop service plans determine eligibility for services and coordinate care while maintaining accurate documentation.

Responsibilities
  • Provide social service support to individuals and families requiring assistance.

  • Conduct client assessments and identify social service needs.

  • Develop implement and monitor individualized service plans.

  • Determine eligibility for social medical and community assistance programs.

  • Coordinate resources and referrals to appropriate agencies and services.

  • Assist clients with access to public assistance food assistance healthcare and supportive services.

  • Perform counseling and support activities as appropriate.

  • Maintain accurate and timely documentation of case activities.

  • Collaborate with interdisciplinary teams to ensure continuity of care.

  • Monitor client progress and update service plans as needed.

  • Utilize EPIC CarePort or other case management systems as applicable.

  • Ensure compliance with organizational policies confidentiality standards and regulatory requirements.

Required Qualifications
  • Bachelors Degree from an accredited college or university.

  • Minimum 3 years of case management social services or related experience.

  • Strong communication and interpersonal skills.

  • Excellent organizational and documentation skills.

  • Ability to work independently and manage multiple cases.

  • Proficiency with computer systems and electronic documentation.

Preferred Qualifications
  • Hands-on experience with EPIC and/or CarePort.

  • Experience working in healthcare social services or community-based settings.

  • Knowledge of public assistance and social service programs.
