Case Worker, Behavioral Care, PRN, Days

Essential Functions

• Evaluates level of care for appropriateness and communicates pertinent clinical information to physicians and members of the interdisciplinary as needed.

• Collects and prepares reports on data requested for case management and/or quality purposes and reports data as directed.

• Pursues organizational goals: reduced LOS improved patient and team member experience and quality metrics in the acute care environment.

• Coordinates care and educates physicians and other staff to facilitate the effective use of clinical and financial resources focusing on admission concurrent/continued stay and discharge planning needs in an effort to obtain acute care certification for the patient.

• Serves as a resource to physicians social workers and other staff for quality and utilization issues in the acute care environment as well as discharge planning needs.

• Performs other duties as assigned.

Supervisory/Management Responsibilities

• This is a non-management job that will report to a supervisor manager director or executive.

Minimum Requirements

• Education - Bachelors degree in Nursing Social Work or Counseling

• Experience - Five (5) years of experience in Behavioral Health
