Job Title: Case Manager / Social WorkerLocation:Golden Valley MN or Hopkins MN. Travel throughout the Twin Cities is required for meetings and visits with persons served. Based on caseload travel throughout other parts of Minnesota may be : Monday-Friday typical daytime business hours. Eligible to work from home up to 2 days/week after 6 months of : Starting at $51000 annual salary including Full-Time benefits PTO and hybrid annual salary available with Bachelors or Masters of Social Qualifications:Be a licensed Social Worker (Bachelor of Social Work required for licensure)Or licensed Graduate Social Worker (Masters in Social Work required for licensure) as stated by the Minnesota Board of Social WorkOr have a 4-year degree in Human Services Psychology or Sociology or related fieldsSuccessful clearance of Department of Human Services background checkSuccessful clearance of Motor Vehicle Background Check and acceptable driving record per Company PolicyJob Summary: As a Case Manager you will be working on finding resources and services for persons (clients) that best fit their needs and situations. At Meridian Services each Case Manager is an advocate for persons with disabilities/elderly and helps to make a difference in their lives. Our Case Managers will carry caseloads that consist of persons with intellectual disabilities traumatic brain injuries CADI (mental health) diagnoses and elderly/alternative Job Duties:Visits with persons on the caseload and attending team meetingsCompleting or attend meetings on needs assessment; individual service plan development (CSSP)Assisting with planning for new service developmentLocating residential vocational and other needed services depending on each persons needsMonitoring service delivery and ensuring the health and safety needs of each person are being of referrals and crisis intervention as neededDeveloping waiver budgetsCompleting case notesActing as an advocate for people we provide services toPreferred Qualifications:Knowledge of community resources and providersExcellent computer skills including Microsoft officeGreat organizational skills and attention to of Medical Assistance Medicare and MN Health care programs
