Position Summary

Under the general supervision of the LifeSpring Program Manager provides intensive case management and independent living skills development to foster youth and probation youth transitioning into independent living. Assist youth in developing skills obtaining employment pursuing educational goals and developing life plans that will support independence.

Services are provided in a client-centered trauma-informed and culturally sensitive manner that is consistent with Lifelines philosophy and mission to build self-sufficiency. Possess knowledge and expertise in relationship to transitional age youth specifically former foster youth and the specific issues surrounding this population and demonstrate excellent engagement skills.

Provide skill development opportunities for youth including life skills training meditation economic literacy transportation assistance public benefits advocacy financial assistance and tenancy training. Provide supportive services to parenting youth including individualized housing assistance securing childcare education and support maternity/paternity provisions and specialized services for children.

Provides linkage to services within the community to support transition to independence as well as aftercare including but not limited to mental health services educational (college or post-secondary training) and employment support etc. Provide crisis management and safety planning to the youth in the program.

Case management will have a therapeutic component and evidenced based practices should be utilized. Participation in county collaborative meetings to identify service gaps barriers and successes in order to create best practices policies procedures and protocols. Provide training and outreach and identify and seek partnerships with other agencies and organizations for collaboration.


Qualifications

  • BA in social work or related field required
  • Masters and BBS registered intern preferred.
  • A minimum of 3 years relevant experience require
  • Experience working with teens and/or young adults including transitional age youth specifically former foster and probation youth preferred.

Specific Activities

Duties include but are not limited to:

  • Utilize engagement strategies to build rapport identify strengths and barriers to participant self-sufficiency and empower youth to achieve goals and permanent independence.
  • Provide intensive case management services designed to build and support participants relationships with family and community with an emphasis on permanent connection
  • Utilize evidenced based practices to engage youth in participation of services that are client-centered culturally appropriate trauma-informed in order to improve the short/long-term health safety and well-being of youth.
  • Utilize evidence based practices as the modality for crisis management and safety planning as well as individual and group sessions.
  • Provide coordination of care with other mental health professionals primary care providers and other supportive services.
  • Coordinate and engage youth in activities in the community to stimulate alternative interests improve emotional and physical wellbeing foster leadership development enhance self-esteem and support personal growth and satisfaction.
  • Provide support advocacy life skills referrals crisis intervention and employment readiness
  • Carry a caseload of youth as required by funding source and provide responsive services in the office home and/or community setting.
  • Provide outreach and marketing to prospective participants and community partners
  • Work closely with Housing Specialist to screen and interview prospective participants
  • Assist youth in obtaining documentation housing education and legal services etc.
  • Assess appropriate applicants and co-create an individualized case plan monitoring the case plan and providing assistance as needed.
  • Process assistance requests for participants as needed including but not limited to taking residents
  • Maintain accurate timely documentation for each client including tracking short and long term client progress in Efforts to Outcomes (ETO) as required by contra
  • Assist in program development and resource identification maintaining linkages with community educational and employment resources.
  • Collaborate with County ILS workers ILS contractors social workers probation officers and other required parties in development and delivery of service plans.
  • Participate in multi-disciplinary teams serving as an expert on matters related to behavioral health.
  • Attend and participate in program meetings group supervision retreats committees trainings and other collaborative activities as necess
  • Maintain confidentiality and privacy standards of participants protected health information and other applicable information in accordance with HIPAA.
  • Uphold LifeSpring program compliance and contractual standards through direct monitoring of participants.
  • Ability to work independently and use sound judgment in decision making.
  • Complete all other duties as assigned.

Schedule/ Compensation

  • FT/ nonexempt position
  • Mon-Fri 8 hours a day may include evenings and weekends.
  • Evening and weekend on call shifts
  • MA or BA5- $21.00 - $26.64 MABBS $26.64 - $30.00 Salary will commensurate with experience and education. Differential pay offered for bilingual skills. EEO.

Additional Requirement

  • Must have a valid drivers license and reliable transportation with a good driving record.
  • Willing to use own properly insured reliable vehicle for the job (mileage reimbursed).
  • Employment subject to clearances including background finger printing checks driving record tuberculosis testing and drug testing.
