Role: Case Manager - Time Limited SubsidyReports to: Program managerProgram: 1603-Housing RetentionDepartment: Permanent HousingLocation: The Nest - 222 South Hill St. 7th Floor Los Angeles CA 90012Schedule: Monday - Friday 8:00am - 4:30pmStatus:Full-time/Non-Exempt/Non-ManagementOpenings:1Summary:As part of a multi-disciplinary team the Time Limited Subsidy (TLS) Case Manager will travel to multiple locations in the greater Los Angeles area to meet and work with clients to attain and sustain permanent housing in partnership with the County Department of Homeless Services & Housing (HSH). The TLS Program offers short to mid-term rental subsidies combined with linkages to employment and community-based services in order to end homelessness for participants and support them in achieving permanent housing stability. The TLS Case Managers primary responsibilities are to implement housing stabilization plans and coordinate linkage to community-based services and permanent housing. This program focuses heavily on supporting individuals with increasing their income via linkage to employment resources and appropriate benefits programs as well as life-skill coaching needed to ensure housing retention. Case Managers are also responsible for building and maintaining good working relationships with landlords management companies and property Duties and Responsibilities:Work with a diverse and marginalized client caseload of 20 clients experiencing many barriers including homelessness complex trauma medical mental health and substance useEnsure that case management services to clients are reflective of the Whatever It Takes model which emphasizes customer choice harm reduction trauma-informed care and psycho-social rehabilitationEnsure that each client on caseload has an up-to-date and comprehensive biopsychosocial assessment and that this assessment is used in collaboration with the client to create individualized case management plans designed to improve quality of life and improved health outcomesCollaborate with each client to develop creative goal plans aimed at improving overall well-being and housing stability; review and update quarterly and upon completion of goalsDevelop and maintain a complete accurate and current client file with all required documents and data in agency records and electronic databasesDocument all client contacts in PIP format (Purpose Intervention Plan) and in accordance with Housing Department productivity expectationsCoordinate with HSH and community partners to ensure clients are connected to primary health care and insurance to reduce need for emergency health care servicesMaintain confidentiality of client files per HIPAA and all applicable guidelinesResponsible for coordinating appointments transportation and follow-up services for clients accessing primary health care mental health care recovery services and other community resourcesTransport clients in your vehicle to various agencies and healthcare providers in the greater Los Angeles area to increase community support services and community reintegrationDevelop effective trusting relationships with clients with a focus on facilitating independence and maintenance of improved physical and mental healthAssist in the screening assessment and enrollment of clients including orientation to program policies resources and goalsMaintain a current thorough knowledge of community resources and utilize them to provide comprehensive wrap-around services to clientsAssist clients with becoming involved in daily activities scheduling and attending appointments budgeting socialization health maintenance sense of community and progress in recoveryCollaborate with Brilliant Corners (BC) and submit accurate and timely documentation for monthly financial assistance requests. Ensure all relevant documentation is provided to BC in a timely with completion of housing applications for individuals who are eligible for Permanent Supportive Housing cooperatively and cohesively with other clients of the staff team including participation in weekly staff meetings and staff duties as assignedQualifications:Valid CA Drivers license reliable car auto insurance and an acceptable driving and able to transport Clients in your personal degree preferred; or a minimum two (2) years case management experience working with individuals experiencing homelessness preferably those living with mental illness and/or substance addictions or an equivalent combination of education and experienceDemonstrated knowledge of case managementAble to work in a high tolerance Harm Reduction model with clients who have multiple barriersSkill in non-violent crisis interventionComputer literate; able to effectively use computerized database for client file management with basic skills in Microsoft Word Outlook and Excel programsDetail oriented with strong time management organizational written verbal interpersonal and computer skillsAble to obtain and maintain CPR/First Aid certificationPreferred Qualifications:Minimum 30 WPM typing speedBachelors DegreeWork Environment:Combination of field and office required to sit stand bend; occasionally lift or carry up to 35 occasion walk or drive to different local necessitate working in busy and loud be exposed to elements like cold heat dust noise and need to bend stoop twist and sit throughout the day.
