The primary goal of this role is to support participants in becoming document-ready and successfully transitioning to permanent housing as quickly as possible through coordinated case management problem-solving and resource Description Essential Duties and Responsibilities: Provide participant-centered case management using Housing First harm reduction and trauma-informed care initial and ongoing needs assessments to identify barriers to housing and monitor individualized housing and service with assigned participants at least monthly or more frequently based on participants in becoming document ready for housing participant progress toward permanent housing participants with obtaining vital documentsHelp complete applications and accompany participants to appointments when in resource coordination and case conferencing with CES partners and community providers to facilitate housing problem-solving conversations throughout enrollment to identify diversion or housing timely accurate and objective case notes in program enrollments updates and exits within one business day of service complete and compliant participant all engagement efforts and participant participants to mainstream benefits (DPSS CBEST DMH SAPC etc.).Link participants to employment and workforce crisis intervention and conflict de-escalation support as harm reduction practices and overdose prevention required staff meetings trainings and case appropriate releases of information for coordination of a safe low-barrier and participant-centered program recommended caseload ratio of approximately 1:25 participants (or as assigned).Demonstrate consistent progress of participants toward permanent timely and compliant HMIS Housing First and trauma-informed service intakes for individuals who are referred into interim housingCreate a supportive environment and ensure clients progress utilizing a strengths-based approachFacilitate wellness enrichment and life skills groupsStaff must transport client in personal vehicleOther duties as assignedJob Description Qualifications:High School Diploma or GED or EquivalentValid CA Drivers license reliable car auto insurance and an acceptable driving and able to transport Clients in your personal working with participants with mental illness chronic health issues and substance abuse disordersWillingness to work in a high tolerance Harm Reduction model with clients who have multiple barriersExperience with Outreach and engagement crisis intervention suicide assessment/prevention affordable housing housing rights human relations/communication skills; ability to act in professional manner regardless of circumstancesDetail oriented with excellent time management skillsAbility to self-motivate and be flexible in a fast-paced environment with minimal supervisionStrong verbal and written communication skills including knowledge of Microsoft Suite (Word Excel Outlook etc) requiredPassion for working with the population servedAbility to work as part of an interdisciplinary teamJob Description Preferred QualificationsBachelors degree; or a minimum two years experience working with homeless individuals preferably those living with mental illness and/or substance addictions or an equivalent combination of education and of one (1) year experience in case management preferredKnowledge of Housing First harm reduction and trauma-informed care using HMIS or similar database documentation and organizational to work effectively with diverse and high-acuity populationsJob Description Work Environment:Interim housing site with high-acuity participantsCombination of office field and on-site workMay require evenings/weekends as assignedOffice environment with occasional field activities with participants and required to sit stand bend and occasionally lift or carry up to 35 be exposed to elements like cold heat dust noise and need to bend stoop twist and sit throughout the day
