As our Case Manager Team Lead you will direct plan organize and lead all activities of the Department staff during a shift ensuring timely efficient processes and the highest quality of professional care.

Every day you will ensure high standards are met to serve the needs of patients physicians regulatory agencies community agencies and all hospital departments fostering exceptional service and compliance.

To be successful in this role you will possess strong leadership and organizational skills a deep understanding of case management processes and the ability to drive quality and efficiency while maintaining professional standards and compliance.

• Obtains reviews and analyzes admission information per hospital policy documenting assessments proactively reviewing medical records for compliance assessing and coordinating resource use communicating plan modifications facilitating family conferences and prioritizing observation patient throughput.
• Daily review of all clinic bed observation 1-day inpatient stays and same-day discharge charts for appropriate status orders and calculated observation hours proactively managing length of stay participating in Utilization Review meetings and obtaining required regulatory documentation.
• Facilitates discharge planning using ECIN working with patients families and the treatment team to make referrals/arrangements and document actions.
• Identifies and tracks variances in the anticipated plan of care for process improvement referring to CMO or PA for peer review and uses established criteria for level of care medical necessity and continued stay enlisting Physician Advisor involvement as needed.
• Assesses/reassesses patients clinical and psychosocial status diagnosis and treatment plan involving and communicating with physicians as needed and documents Case Management actions and discharge planning promptly in the electronic health record.
• Assists in directing organizing and prioritizing all shift activities ensuring patient physician and staff needs are met problem-solving immediate issues demonstrating a positive role model and assisting in staff hiring orientation development and performance recommendations.

• Graduate of accredited school of nursing or Masters of Social Work
• Four (4) years as a registered nurse or four (4) years as a social worker
• If Registered Nurse current state RN license
• BLS certification from the American Heart Association
