Job Summary
Case Manager needed for full-time temporary position in South Philadelphia.
Responsibilities include completing intake assessments developing and monitoring individualized service plans maintaining case files and connecting residents to medical mental health housing and social services. The Case Manager assists residents with appointments housing applications and transportation as needed and documents progress outcomes and housing activities.
The role involves collaboration with the Program Director/Supervisor to approve admissions facilitate volunteer-led life skills groups schedule resident responsibilities and conduct regular check-ins including follow-up with former residents. Staff participate in training flexible schedules evening and weekend activities and monthly on-call rotations.
