The Home Care Coordinator Discharge Planner specializes in coordinating home care services and resources for patients transitioning from hospital to home acting as a discharge planning resource without providing hands-on clinical care.

Work Location: Anne Adrundel Annapolis Maryland

Weekly Working Hours: Days 24 hours per week

Qualifications:

• Bachelor of Science in Nursing (BSN) required
• Licensure in clinical discipline (RN)
• Active CPR
• Two (2) years experience in clinical nursing required previous home care or home care discharge planning experience preferred
• Knowledge of Medicare Conditions of Participation preferred
• Thorough knowledge of home care community resources agency policy and procedures federal and state regulations and third party regulations for reimbursement of home care services preferred.

Salary Range: Minimum 39.64/hour - Maximum 61.45/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount salary discussions will occur during candidate screening calls before any subsequent compensation discussion is held between the candidate and any hiring authority.
