What you will do: Responsibilities listed include but not limited to:

• Work with insurance and payers to advocate for the appropriate rehabilitation and medical care.
• Conduct comprehensive assessments of patients medical social and psychological needs
• Collaborate with the treatment team to develop individualized care plans.
• Coordinate and monitor the delivery of services including therapy medical care and support services
• Advocate for patients rights and ensure their needs are met throughout their rehabilitation journey
• Provide education and support to patients and their families

What you will need:

• Bachelors or Masters degree in a related field preferred (e.g. Social Work Psychology)
• Clinical case management experience preferably in a rehabilitation or healthcare setting
• Strong interpersonal and communication skills
• Knowledge of brain injuries and rehabilitation preferred
