We are currently seeking a passionate Case Manager / Housing Specialist to join our PATH (Projects for Assistance in Transitioning from Homelessness) Program. This position is ideal for professionals with experience in case management social services behavioral health human services homelessness services supportive housing community outreach and client advocacy.
Position Summary
The PATH Case Manager provides community-based case management outreach housing navigation advocacy and recovery support services to individuals experiencing homelessness and serious mental illness.
The successful candidate will assist clients in obtaining stable housing healthcare behavioral health treatment public benefits community resources supportive services and recovery-oriented care while promoting long-term stability and independence.
This role combines field outreach client engagement care coordination service planning crisis intervention resource linkage housing assistance benefits navigation and recovery support.
Key Responsibilities
Conduct community outreach and engagement with individuals experiencing homelessness or housing instability.
Complete assessments screenings and service planning activities.
Develop and implement Individual Recovery Plans (IRPs) and person-centered care plans.
Provide ongoing case management and care coordination services.
Assist clients with securing permanent housing supportive housing and emergency housing resources.
Connect individuals to mental health substance use medical vocational educational and community support services.
Provide advocacy and assistance with entitlement programs public benefits and community resource navigation.
Collaborate with treatment providers family members support networks landlords housing agencies and community partners.
Deliver supportive interventions utilizing Motivational Interviewing (MI) and other evidence-based engagement strategies.
Assist in reducing barriers to treatment housing stability employment and recovery.
Document services accurately and maintain compliance with agency state and federal program requirements.
Participate in multidisciplinary team meetings and community partnerships.

Required Qualifications
Bachelors Degree from an accredited college or university in:
Social Work or Related Human Services Field
Minimum two (2) years of experience in:
Case Management

Knowledge of housing resources community agencies supportive services and public benefit programs.
Strong communication engagement advocacy and relationship-building skills.
Valid drivers license with acceptable driving record.
Preferred Qualifications
Experience working with individuals experiencing homelessness serious mental illness co-occurring disorders or housing instability.
Knowledge of PATH programs supportive housing Housing First models and community-based behavioral healthcare.
Experience in crisis intervention recovery support and care coordination.
Bilingual English/Spanish strongly preferred.
Experience utilizing Motivational Interviewing (MI) and trauma-informed approaches.
