As a PATH Case Manager youll work directly with individuals experiencing homelessness and serious mental illness to help them obtain stable housing and supportive services.

Key responsibilities include:

• Conducting community outreach and engagement activities
• Developing Individual Recovery Plans (IRPs)
• Providing ongoing case management and care coordination
• Assisting clients with permanent supportive and emergency housing resources
• Connecting individuals with mental health medical substance use vocational and educational services
• Assisting with public benefits and entitlement programs
• Utilizing Motivational Interviewing (MI) and recovery-oriented interventions
• Maintaining accurate clinical documentation and compliance requirements
• Supporting recovery and long-term independence

• Bachelors Degree in Social Work or a related Human Services field
• Minimum two years of experience in fields such as:
  • Case Management
  • Behavioral Health
  • Mental Health Services
  • Social Services
  • Homeless Services
  • Human Services
  • Community Outreach
• Knowledge of housing resources and public benefit programs
• Strong communication advocacy and relationship-building skills
• Valid drivers license with acceptable driving record

• Experience working with individuals experiencing homelessness or housing instability
• Experience supporting individuals with serious mental illness or co-occurring disorders
• Knowledge of Housing First and supportive housing models
• Experience with crisis intervention and care coordination
• Bilingual English/Spanish
• Experience utilizing Motivational Interviewing and trauma-informed care
