JOB SUMMARYHealthRIGHT 360 a nonprofit organization and a family of programs is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services including mental health care residential and outpatient substance use treatment and primary health services. Additionally we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health we empower individuals to overcome challenges by addressing social determinants of health fostering resilience and facilitating Prototypes Womens Center in Oxnard offers residential substance abuse treatment and detox services tailored specifically for women including those who wish to involve their children in their journey to recovery. Our program in Ventura County aims to assist women in maintaining their progress in overcoming co-occurring disorders through a wide range of supportive services. Children can foster a secure attachment with their mothers in a secure setting while also accessing mental health support and educational assistance. The facility can accommodate up to 56 women and 28 children for Case Manager III delivers in-person client services by coordinating care within the organizations continuum of services and with external human services providers. They collaborate closely with clients to assist in completing intake paperwork outcome assessments and applying for benefits. Working alongside a multidisciplinary team Case Manager III ensures optimal treatment outcomes aligned with HealthRIGHT 360s philosophy goals policies mission and vision. Services are provided flexibly across outpatient offices residential treatment settings or in the community as dictated by client needs and program requirements. Therefore a reliable transportation is RESPONSIBILITIESDirect ServiceAssist the client with intake by completing case management assessment and entering financial and benefit the client in applying for Medi-Cal or transferring Medi-Cal County when the clients case management needs and complete all releases of the client to benefits mental health physical health employment probation DCFS housing community resources outpatient substance use disorder services and communication and external service linkage including assisting with scheduling appointments communicating with probation scheduling child visits communicating with DCFS obtaining all court minute orders providing appointment reminders for therapy and services in outpatient residential or field depending on assignments and client the VI-SPDAT and connect the client to safe housing the client for job search by assisting with resume writing and job search clients progress toward achieving case management treatment plan goals and provide treatment plan clinical consultation throughout the treatment episode and when encountering new and/or high-risk clinical collaboratively with other members of the treatment team including the Clinical Therapist Counselor Medical and collaboratively with all members of the behavioral health team including medical mental health psychiatry substance use disorder and other clients as client UA tests and document test clients in developing independent living skills to promote independence and and complete all progress notes within 72 hours of service compliance with the due dates for all clinical clients progress letters and court reports and obtain final approval from the all documentation in compliance with agency policy and procedures HIPAA 42-CFR DMH and other standards to satisfy internal and external all assigned URCs within a timely and Other DutiesAssist with client program oversight by managing case management business expected client care full compliance with credential and actively participate in assigned participate in in training opportunities and complete all assigned training in a timely work schedule in accordance with the agencys emails daily and respond when with onboarding and training of new employees interns ambassadors volunteers etc. or as perform other duties as and ExperienceOption I:High school diploma or Use Disorder Counselor certification from an accredited California agency (CCAPP CAADE CADTP).Option II:Bachelors degree in Social Work Psychology or other related field from an accredited Use Disorder Counselor registration from an accredited California agency (CCAPP CAADE CADTP).At least two years case management experience preferably with clients experiencing acute withdrawal from in the human service field and demonstrated expertise in substance abuse treatment relapse prevention and First Aid and CPR certification or ability to obtain within 30 days of California Drivers License and access to registered and insured transportation.
