JOB TITLE: Case Manager

EMPLOYMENT TYPE: Full-time 40 Hours

SCHEDULED HOURS: Monday-Friday 8am-4pm

PROGRAM/LOCATION: Oak Street Willimantic CT

ABOUT THE PROGRAM: Oak Street

10 Bed Co-Ed Transitional Living Program for adults with Substance Use Disorders in Willimantic. Clients must attend outpatient treatment while in the program. Clients must have a discharge plan as it is a transitional living. Clients with co-occurring disorders and on MAT are accepted.

ABOUT THE POSITION: Case Manager

Duties & Responsibilities

    Utilizes appropriate therapeutic models to aid clients in understanding and managing their symptoms in a variety of environments. Conducts program specific assessments and screens as required by program
    Provides case management. Works in close collaboration with the clients natural supports and treatment teams. Connects to prosocial activities. Provides crisis intervention and de-escalation as needed.
    Demonstrates competency in age related or specialty issues and developmental needs for each population served. Demonstrates program specific target population competencies.
    Demonstrates the ability to teach and support the learning of a broad range of skills including those related to emotion regulation age-appropriate activities and competencies and living in a home-like setting. Plan and actively participate in activities to support and increase skills and competencies.
    Documents all services and contacts with clients collateral contacts and appointments in compliance with agency standards.
    Transports clients as required by program.
    Participates in staff meetings supervision training and other meetings as assigned.
    Additional duties as required.

      QUALIFICATIONS:

      Education: Associates required plus bachelors equivalent experience (2 years)

      Experience: At least two (2) years experience in a mental health setting preferred.

      Licensure/Certification/Registration: Valid drivers license.

      COMPENSATION*:

      $19.2850/hour min- $26.9735/hour max

      *Actual rates within this range are determined at the time of offer and are based on relevant experience as well as adherence to our internal equity policy. (Please note for part-time positions this range would be pro-rated based on hours worked)
