CrossNet Ministries is a Christ-centered nonprofit offering health hope and practical support in the name of Jesus to individuals and families throughout Eastern Lancaster County. Through more than 30 community programs and strong partnerships with local churches schools and organizations we connect people with resources and relationships that help them thrive. What Youll Do as a Case Manager
- Provide trauma-aware strengths-based case management to individuals and families managing an average of 15 cases each month.
- Conduct participant intakes assessments and ongoing check-ins to identify needs establish goals and track progress.
- Develop individualized service plans and connect participants with CrossNet programs community resources and external agencies.
- Document participant interactions referrals services and outcomes while maintaining confidentiality and accurate records.
- Collaborate with other Case Managers program staff and leadership to coordinate participant care gather data and support reporting needs.
- Improve Participant Services workflows intake processes and documentation systems by helping identify trends gaps and opportunities.
- Support CrossNet programs and community initiatives including food distribution emergency assistance outreach and seasonal programs.
- Represent CrossNet Ministries during local home visits community meetings and partner collaborations throughout Eastern Lancaster County.
Our Ideal Case Manager
- Experienced: Bring 2 years of experience in case management social services or a community support role.
- Educated: Hold a bachelors degree; Social Work Human Services Psychology or a related field is preferred.
- Compassionate & Relational: Approach individuals and families with patience and empathy building trust while navigating complex situations professionally.
- Collaborative & Adaptable: Work effectively as part of a team while managing independent responsibilities and adapting to evolving program and participant needs.
- Organized & Dependable: Maintain accurate records manage participant data protect confidentiality and demonstrate strong attention to detail and follow-through.
- Faith-Driven & Ethical: Demonstrate ethical practice respect confidentiality and embrace CrossNet Ministries Christ-centered mission of serving the ELANCO community through practical care and the love of Jesus.
