Job Summary
Odyssey HouseLouisiana Inc. is seeking a full-time Case Manager. The schedule is Monday-Friday 8:30am-5:30pm. The Case Manager position is responsible for determiningeligibility for guests to participate in the Agencys Low Barrier Shelter(LBS). The Case Manager will utilize Motivational Interviewing to assess theguests needs/disabilities to create an effective individualized housing plan.
Responsibilitiesand Duties
It is the duty and responsibility of the Case Manager to:
- providecase management to approximately thirty or more guest(s) as assigned; Case
- Managementwill involve screening intake assessment individualized housing plandevelopment monitoring linkage to appropriate community resources follow upappropriate transition to housing etc.;
- assistguest(s) with obtaining vital records ID cards Drivers Licenses and otherdocumentation;
- assistguest(s) with completing lease agreements and obtaining other supportive
- documentssuch as identification social security card etc. as required by leasingagents/property;
- assessguest(s) Medicaid status and follow agencies policy and procedures to obtainand maintain Medicaid status;
- enterguests information into the appropriate agencys database(s) in a timelymatter and update as needed;
- prepareand submit reports to Project Manager in a timely manner;
- maintainup to date information regarding tenant occupancy move outs and vacancies
- utilizeMotivational Interviewing (MI) a client-centered approach for elicitingbehavior change by guiding participants to explore and resolve ambivalence;
- ensure that guest(s) are referred toand/or aware of available vocational educational housing and social servicesin the community;
- assistguest(s) (whether through direct intervention or referral to appropriateservices) in the development of life skills necessary to: navigate thesurrounding environment; access community resources such as banks employmentagencies government offices and recreational and educational organizations;serve as a resource to the community; pursue educational and occupationalopportunities; obtain housing; manage finances; and access public assistance;
- attend all required meetings andtrainings;
- keep written progress notes of workdone on the case;
- make necessary referrals to fillpotential gaps in assistance;
- verify the results of the assistanceprovided in order to close the case as per the approval of the ProgramManager;
- provide crisis management whenindicated by guest(s) needs;
- complete all Incident Reports prior tothe end of scheduled shift as required by Incident Policy and Procedure;
- maintain guests licensingrequirements;
- oversee daily guest activities forguest(s) in the LBS; and
- perform other duties as assigned.
Qualificationsand Skills
- Valid drivers license
- Excellent communication andorganizational skills
- At least two years of case managementexperience
- Database management experience
- Ability to work with homeless guests of diverse backgrounds and disabilities including physical mental substance abuse immunodeficiencies etc
Preferred
- Bachelors Degree
- Experience with Target population
- Strong written and verbalcommunication skills in both English and Spanish
