Make an impact as a Case Manager by leading patient-centered care coordination from admission through this role you will manage a dynamic caseload assess patient needs and collaborate with physicians care teams and families to develop and execute effective cost-conscious care plans. Youll play a key role in streamlining care delivery removing barriers and ensuring smooth transitions across the continuum of care.

32 Weekly Hours Full Time Dayshift (will work rotating weekends)
Requirements:
Education:
- Bachelors degree required (Equivalent combination of education and relevant clinical experience may be considered in place of a degree)
Experience:
- Minimum of 3 years of job-related experience required (Experience in an acute care setting required preferably within the area of practice)
Licensure/Certification:
- Active RN license with the Kentucky Board of Nursing or a valid compact RN license
- Case Management Certification (ANCC CCM or ACM-RN) - Required within 2 years of hire.
