Job Title:Case Manager Jail Program (ODR) - Sherman WayDept:Project-Based Housing DepartmentReports To:Senior Program ManagerStatus:Full-Time; Non-Exempt (Hourly); Non-ManagementSchedule: Monday-Friday 8am-4:30pmLocation:Sherman Way 13561 Sherman Way Van Nuys CA 91405Benefits: Medical Dental Vision Retirement Planning (403b) Employee Assistant Program (EAP) :In partnership with the Department of Health (DHS) and the Office of Diversion and Re-entry (ODR) Team the Case Manager will provide Intensive Case Management Services (CMS) to clients recently released from jail who are experiencing homelessness. The Case Manager will assist with coordinating clients release and help them attain permanent supportive housing in the community. The ODR Case Manager will work with clients to provide intensive field-based case management to a diverse group of clients with complex trauma facing medical mental health and substance use barriers. Clients remain in the program for as long as they require subsidized Duties & Responsibilities:Work with a diverse and marginalized client caseload of 15 justice-involved clients experiencing many barriers including history of homelessness complex trauma medical mental health and substance useMaintain weekly contact with all clients and document contact in County approved databaseEnsure that case management services to clients are reflective of the Whatever It Takes model which emphasizes customer choice harm reduction trauma-informed care and psycho-social rehabilitationLink clients to appropriate medical mental health substance abuse and vocational resourcesAssist clients in obtaining all benefits to which they are entitled6. Participate in court hearings and submit court progress reportsEnsure that each client on caseload has an up-to-date and comprehensive biopsychosocial assessmentCollaborate with each client to develop creative goal plans aimed at improving overall well-being and housing stability; review and update quarterly and upon completion of goalsDevelop and maintain a complete accurate and current client file with all required documents and data in agency records and electronic databasesDocument all client contacts in PIP format (Purpose Intervention Plan) and in accordance with Housing Department productivity expectationsMaintain confidentiality of client files per HIPAA and all applicable guidelinesResponsible for coordinating appointments transportation and follow-up services for clients accessing primary health care mental health care recovery services and other community resourcesTransport clients in your vehicle to various agencies and healthcare providers in the greater Los Angeles area to increase community support services and community reintegrationDevelop effective trusting relationships with clients with a focus on facilitating independence and maintenance of improved physical and mental healthMaintain a current thorough knowledge of community resources and utilize them to provide comprehensive wrap-around services to clientsAssist clients with becoming involved in daily activities scheduling and attending appointments budgeting socialization health maintenance sense of community and progress in recoveryWork cooperatively and cohesively with other clients of the staff team including participation in weekly staff meetings and staff trainingsQualifications:Valid CA Drivers license reliable car auto insurance and an acceptable driving recordAble to pass a background check for jail clearanceAble to transport clients in your vehicleBachelors degree preferred; or a minimum two years case management experience working with individuals experiencing homelessness preferably those living with mental illness and/or substance addictions or an equivalent combination of education and experienceDemonstrated knowledge of case managementAble to work in a high tolerance Harm Reduction model with clients who have multiple barriersSkill in non-violent crisis interventionComputer literate; able to effectively use computerized database for client file management with basic skills in Microsoft Word Outlook and Excel programsDetail oriented with strong time management organizational written verbal interpersonal and computer skillsAble to obtain and maintain CPR/First Aid certificationPreferred Qualifications:Minimum 30 WPM typing speed preferredBachelors DegreeWork Environment:Combination of field and office environmentRegularly required to sit stand bend; occasionally lift or carry up to 35 occasion walk or drive to different local sitesWill necessitate working in busy and loud environmentsWill be exposed to elements like cold heat dust noise and odorMay need to bend stoop twist and sit throughout the day
