For over 125 years Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens and currently offers 160-plus programs and services for children youth adults seniors and those struggling with mental illness. Older Adult Centers promote an active life for seniors. The centers aim to assist them to preserve their independence and health by offering various range of activities and services such as serving breakfast lunch health screenings social services educational workshops class exercises discussion groups games art classes lectures workshops organized outings and special OF THE JOBUnder the direction of the Program Manager the Case Manager II determines the client needs for assistance arranges for services provides information and referral and assists the Program Manager to plan and coordinate education/recreational activities for the needs and provide information referral and case assistance to seniors. Develop an appropriate care plan and follow through to disposition. Maintain records case files and statistics regarding client files and activities according to DFTA and agency a group discussion at least monthly benefits information workshops for seniors. Coordinate special events and activities as assigned by the Program :Bachelors Degree in human-related services from an accredited college or of entitlement for older adults in computer software applications Excel and Microsoft preferredAble to travel to clients homes meetings conferences and other locations for training. Ability to lift to 10lbs.
