Job Summary

For over 125 years Catholic Charities Brooklyn and Queens have been providing quality social services to the neighborhoods of Brooklyn and Queens and currently offers 160-plus programs and services for children youth adults seniors and those struggling with mental illness. Healthy Families provides participants with information and referrals to entitlement programs education employment assistance access to legal assistance housing transportation and mental/health services including HIV/AIDS prevention and treatment. Staff arrange appointments for clients conduct assessments screen for benefits and entitlements complete application forms and assist with English/Spanish OF THE JOB:Under the direction of the Program Director the Case Manager I (Family Development Coach) renders professional intervention to clients in need of assistance through comprehensive support services (outreach assessment intake application assistance connection to services and resources to meet their goals and follow-up). Provide case management services assessment and intake of participants to determine eligibility for services including responding to telephone and walk-in inquires conduct pre-screenings and provide brief and full services. Plan and implement workshops to provide information on various topics of concern to the participants as outlined in our contractCarry a caseload required to meet funding source targets. Work out of multiple sites as assigned; conduct outreach make field visits regularly and consistently as per effort to target engage and outreach routinely to publicize the availability of the program to make persons at risk aware of Healthy Families and other available services and and linkage services for participants when and/or assure that each case assigned receives regular communication to complete and maintain case records including completing progress notes. Ensure all relevant case related information is inputted into case files and corresponding :AA degree in social services or related field with two years experience in a social service setting or in providing direct services to individuals or families or Bachelors degree with no experience Development Credential (FDC) must receive certification within 12 months of skills (Spanish) required2 years experience in the NDA (Bushwick/Jamaica) providing access services and experience with diverse to work flexible hours including weekends evenings according to the needs of program to conduct home/field visits and travel throughout the five boroughs as needed.
