Job Summary

STATEMENT OF THE JOBUnder the direction of the Program Manager the CRED Case Manager I renders professional intervention to clients in need of assistance through comprehensive support services (engagements assessment intake application assistance connection to services and resources to meet their goals retention and follow-up).Provide case management services assessment and intake of participants to determine eligibility for services including responding to telephone and walk-in inquires conduct pre-screenings and provide referrals for those not initial assessment to identify participants strength need and goals in collaboration with the mental health counselor job developer and outreach/retention individualized case plans with participants outlining goals objectives and strategies for ongoing support and guidance to participants addressing barriers and challengesFacilitate connections to community resources services and support networks. Refer participants to relevant resources services and participant progress adjusting case plans as needed.
