Competitive Pay

Starting salary is based on experience: $26.00/hr. ($49000/yr) with no related experience up to $32.50/hr. ($61200/yr) with 10 years in the same role
Hourly role paid bi-weekly

Service and Support Administrator (SSA)

As a Service and Support Administrator (SSA) you will serve as a case manager for individuals with developmental disabilitieshelping them access services coordinate supports advocate for their needs and work toward the goals that are most important to them.

A typical day includes meeting with the individuals on your caseload in the community. You might meet at their home a coffee shop one of our offices or wherever they feel most comfortable. Throughout the day youll coordinate services connect individuals with community resources communicate with families and providers and collaborate with your team to ensure each person receives the support they need.

Youll make phone calls using the iPhone we provide attend Zoom meetings with your team and service providers document case notes and update person-centered planning documents on the laptop we provide. Part of your day may be spent working from home completing documentation and coordinating services while the rest is spent in the field meeting with individuals and community partners. You may also stop by one of our offices to pick up supplies or work between appointments.

Job Duties (Summary):

Facilitates individual and family service plan development assigning responsibilities among team members and coordinating goal implementation.
Assists in identifying selecting and obtaining/maintaining services support and benefits; serves as the primary contact for assigned individuals and families.
Ensures timely and accurate documentation adherence to regulatory requirements with supervisory support.
Monitors individual plans for compliance and service quality communicating rights to individuals.
Prepares case notes reports evaluations and summaries while maintaining confidentiality.
Collaborates with staff to problem-solve identify resources and address service gaps.
Complies with relevant state and local guidelines for reporting abuse neglect and incidents.
Attends meetings and conferences for best practices participating in relevant division agency local and state meetings.
Maintains required licenses and certifications.
Travels for service planning and monitoring in line with agency procedures.
Performs additional duties as assigned.

Requirements:

Bachelors degree required; a degree in psychology special education human services or a related field preferred
One year of experience working with individuals with developmental disabilities in a vocational residential case management or treatment program or equivalent preferred
Ability to travel within Hamilton County as needed
Must be able to pass criminal background checks drivers abstract and various abuser registry checks

Unusual Working Conditions:

Service and Support Administrators are required to drive extensively within Hamilton County to perform their duties. Therefore access to reliable transportation is a necessity and an essential function of this position. Planning meetings will be held in individual/family homes and throughout the community.

Hybrid locations work is performed both away from the office and at Hamilton County Central Campus: 1701 Patricia McCollum Way Cincinnati OH 45237

Employees are expected to travel within the county to meet with individuals and attend in-office meetings as needed. The flexibility to work from home is available when job duties allow.
