Southwestern Behavioral Healthcare is currently seeking a Full-TimeCase Managerto add to our team of Case Manager willprovide community-based case management skills training & development and other treatment services to an adult outpatient population.

WHAT IS THIS POSITION RESPONSIBLE FOR

  • Conduct case management skills training & development and other treatment services according to Southwestern policies.
  • Provide case management services to adults assigned to Outpatient Services including but not limited to communicating with all parties involved in the clients treatment; identifying treatment needs and facilitating all necessary referrals; coordinating and attending collateral meetings with all parties involved in the treatment process.
  • Provide emergency counseling and crisis intervention when necessary.
  • Maintain appropriate records of services to reflect patient progress and document treatment plans and goals.
  • Communicate professionally make referrals and coordinate services with community agencies.
  • Must meet efficiency standards which include chart management and patient contact standards


Qualifications

WHATS REQUIRED FOR THIS POSITION

  • Bachelors degree in Social Work Psychology Sociology Criminal Justice or a related Human Service field.
  • Knowledge of available community resources is preferred.
  • Previous work with the SMI population is highly preferred.
  • Demonstrated proficiency with interpersonal and writing skills.
  • Excellent communication time management and organizational skills.
  • Must have valid drivers license auto insurance that meets the Indiana State minimum coverage requirementand qualify for Companys vehicle liability insurance.
  • Candidates must pass required background checks including county/state checks sex offender registry check and drug screen.
