Central Maine Medical Center (CMMC)in Lewiston is the flagship facility of Central Maine Healthcare. CMMC is a 250-bed not-for-profit Level III Trauma Center offering comprehensive inpatient and outpatient services to the people of central Maine. CMMC provides 24-hour emergency care and medically supervised orthopedic rehabilitation programs. Our rehabilitation facilities are part of our all-in-one single floor orthopedics unit designed to support the initial states of recovery helping patients get back home and get moving again sooner. Central Maine Healthcare covers all of patients physical and mental health needs.

How youll contribute

The Licensed Case Manager for the Rehabilitation Unit is responsible for coordinating patient care to ensure appropriate utilization of resources effective service delivery and compliance with regulatory and payer requirements. This role provides comprehensive case management through assessment care planning implementation and ongoing evaluation of individual patient needs while incorporating patient preferences and promoting optimal outcomes.

The Case Manager serves as a patient advocate supporting care coordination discharge planning and continuity of care across the interdisciplinary team. This position focuses on enhancing the patient experience improving quality outcomes and supporting costeffective care throughout the rehabilitation stay. The Case Manager collaborates closely with clinical teams patients and families to ensure safe timely and well-coordinated transitions of care while aligning with the organizations mission vision and values.

What were looking for

  • Current Registered Nurse (RN) or Social Work licensure or Healthcare professional licensure as Physical Therapist (PT) Speech Language Pathologist (SLP) or Occupational Therapist (OT).
  • Certification in Case Management or Rehabilitation Nursing preferred; for example Commission for Case Manager Certification (CCM); Association of Rehabilitation Nurses (ARN) certification American Case Management Association (ACM) or Board Certification in CM by the ANCC e.g.: RN-BC
  • Minimum of 2 years social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred.
