Job Summary
We are looking for a compassionate and driven Case Manager to join our Day Program team where youll make a meaningful impact on the lives of individuals with intellectual and developmental this role youll guide and support a dedicated team ensuring a safe engaging and person-centered environment. Youll oversee client care facilitate enriching training opportunities and serve as a key advocatebuilding strong connections with families caregivers and community partners to promote success and : El CajonSchedule: Mon- Fri 8a-4:30pPay: The pay range for this position is $24.00 to $24.00Who You Are:A dedicated advocate passionate about supporting individuals with developmental disabilitiesA collaborative leader who mentors and supervises Direct Support Professionals (DSPs)Skilled in person-centered support ensuring clients receive quality care and resourcesOrganized and detail-oriented maintaining accurate client records and compliance with regulationsA Typical Day Will Include:Managing a caseload of clients ensuring individualized care and supportDeveloping and implementing Individual Support Plans (ISPs) that foster independence and inclusionSupervising and training DSPs providing mentorship and performance evaluationsCoordinating enriching program schedules and facilitating goal-setting meetingsEnsuring compliance with Community Care Licensing Title 22 and Title 17 regulationsAdvocating for clients by networking with care providers families and funding agenciesWhat Youll Need to Be Successful:High School Diploma required; Bachelors degree in Social Services or related field preferredSupervisory experienceExperience in case management record-keeping and client evaluationsKnowledge of SDRC referral and funding proceduresFamiliarity with Community Care Licensing and California Code of Regulations Titles 17 & 22Valid California Drivers license and ability to transport individuals as neededWed Love It If You:Have experience using electronic case management systems (Therap preferred)Are Bilingual (English-Spanish)Are passionate about team leadership advocacy and quality assurance
