RESPONSIBILITIES

• Independently manage a caseload
• Conduct comprehensive intake assessments.
• Conduct regular field visits to engage and build rapport with chronically homeless individuals.
• Work with clients to develop goals and service plans.
• Provide referrals and direct assistance to support clients applying and/or attending appointments
• for mental health services medical care substance treatment vital documents benefits
• legal/immigration services and permanent housing.
• Document all interactions with or on behalf of clients monitor and document client progress
• toward service plan goals.
• Maintain familiarity with social services and resources available both on-site and in the
• community.
• Coordinate client care with other service providers; attend regular case conferences with
• clients and other service providers as required.
• Attend regular supervision sessions with direct supervisor.
• Drive agency vehicle to and from client locations and appointments
• Perform additional duties as assigned.

QUALIFICATIONS:

• Bachelors degree in social work or a related field of study
• Candidate must have a valid drivers license which has been held for at least 1 year; in the last 3
• years candidate must have obtained no more than 3 license violation points
• One-year of case management or related work experience preferred.
• Ability to engage and build rapport with populations with high support needs.
• Strong oral and written communication skills
• Detail-oriented and resourceful with strong time management and organizational skills
• Proficiency in Microsoft Office suite and other standard business technology
• Bilingual/multilingual preferred but not required.
• Valid drivers license is required for this position.

PHYSICAL REQUIREMENTS

• Ability to use a computer for prolonged periods.
• Ability to occasionally lift and/or move up to 10 pounds.
• Ability to stand walk or sit for long periods of time.
• Ability to bend and retrieve objects and/or documents.
• Ability to travel in the boroughs of New York City and its adjacent counties via public
• transportation.
• Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of
• weather including inclement weather if required to conduct home visits and/or field work
• TB Test required within the first 120 days of employment.
