RESPONSIBILITIES
Independently manage a caseload
Conduct comprehensive intake assessments.
Conduct regular field visits to engage and build rapport with chronically homeless individuals.
Work with clients to develop goals and service plans.
Provide referrals and direct assistance to support clients applying and/or attending appointments
for mental health services medical care substance treatment vital documents benefits
legal/immigration services and permanent housing.
Document all interactions with or on behalf of clients monitor and document client progress
toward service plan goals.
Maintain familiarity with social services and resources available both on-site and in the
community.
Coordinate client care with other service providers; attend regular case conferences with
clients and other service providers as required.
Attend regular supervision sessions with direct supervisor.
Drive agency vehicle to and from client locations and appointments
Perform additional duties as assigned.
QUALIFICATIONS:
Bachelors degree in social work or a related field of study
Candidate must have a valid drivers license which has been held for at least 1 year; in the last 3
years candidate must have obtained no more than 3 license violation points
One-year of case management or related work experience preferred.
Ability to engage and build rapport with populations with high support needs.
Strong oral and written communication skills
Detail-oriented and resourceful with strong time management and organizational skills
Proficiency in Microsoft Office suite and other standard business technology
Bilingual/multilingual preferred but not required.
Valid drivers license is required for this position.
PHYSICAL REQUIREMENTS
Ability to use a computer for prolonged periods.
Ability to occasionally lift and/or move up to 10 pounds.
Ability to stand walk or sit for long periods of time.
Ability to bend and retrieve objects and/or documents.
Ability to travel in the boroughs of New York City and its adjacent counties via public
transportation.
Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of
weather including inclement weather if required to conduct home visits and/or field work
TB Test required within the first 120 days of employment.
