Position: Case Manager 2

Location: Bridgeport

Salary: $22.00/hour

Schedule: Full time 40 hours 1st shift Monday-Friday

Program Summary:

The Supportive Housing Program (SHP) of Fairfield works with chronically homeless women and families in Bridgeport CT through. The program offers permanent rent subsidy and intensive community-based case management services targeted towards housing a lens of trauma-informed care the program partners with program participants to identify goals and resources that help them achieve long-term stability.

Position Summary:

The Case Manager 2 is responsible for providing home- and community-based case management services to homeless women and families throughout Bridgeport CT. Responsibilities include;

• delivering comprehensive case management services to clients in their homes and in their communities
• conducting intake assessments
• assisting youth in locating available apartments for rent
• working in partnership with participants to develop and implement individual service plans that will progress them to stable housing
• liaison with treatment providers and community resources
• providing transportation to participants as needed
• developing money management skills with caseload
• developing employment readiness skills with caseload
• liaison with landlords and property management staff to support participants continued tenancy
• data entry and documentation
• caseload size will vary between 8-12 cases at any given time

Working within our Homeless Services the Case Manager 2 works with the SHP program staff to provide services to stabilize and reunite families experiencing complex issues such as substance use behavioral health matters and homelessness.

Requirements:

• Associates Degree
• 2 years relevant experience in direct service and case management field
• Valid Drivers License
• Regular and predictable attendance is required

Orientation:

If selected for employment with The Connection newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
