Case Management Supervisor - Early Intervention ProgramAnnual Salary: $49920 plus mileage reimbursement overtime eligiblePosition Status: Full-time (40 hours/week)Typical Schedule: Mon-Fri daytime - may include some light on-call responsibilities after hours and weekendsDo you have several years of experience working in human services including experience as a case manager Bring your clinical and organizational skills to a team-based workplace that puts people a Case Management Supervisor for our Early Intervention program in the San Bernardino area you will supervise cases and manage/supervise the therapeutic case management team for the position works with infants to toddlers (ages 0-3) who have been diagnosed with an intellectual or developmental disability as well as the parents/guardians of the children in the cases within the assigned management team for children (infant-toddler age) with intellectual or development disabilities; provide direction support and counsel to team members every and supervise case management team including performance evaluations scheduling orientation and training as well as making recommendations on decisions for employee hires transfers promotions salary changes discipline terminations and similar actions. Resolve employee problems within position caseloads and assign cases to Specialists and Coordinators; review treatment plans to ensure completeness and accuracy; track all necessary data related to case management performance targets and timeliness of reports; maintain internal and external file individuals served to obtain medical history data and other pertinent information; conduct intake and administer diagnostic tests to identify behavioral and personality traits; document in the development and implementation of the Individual Service Plans (ISP) including annual ongoing with a treatment team including a medical doctor and other specialists in decisions related to changes in a treatment plan amend the plan as directed and arrange additional support as arrange and/or administer therapeutic treatment such as behavior modification developmental milestones social skills and so on based on clinical individuals to supportive services to supplement treatment and counseling as on-call coverage for caseload and participate in rotational emergency coverage and crisis intervention support as supervisory home visits and counsel parents/guardians of children in program as that initial and ongoing RBWO waivers are completed and submitted promptly and that SCORE is updated and accurate at all times if required by : Bachelors Degree in Major/field related to Psychology Sociology Child or Human Development or other health or human services note: We are unable to make exceptions on education - must have completed at least a Bachelors level degreeTwo to four years of experience in human services INCLUDING at least one year of experience as a Case Manager (or similar title as long as primary duties of position are case management related)Must have experience directly supporting children and families in a professional role preferably in a therapeutic capacityValid CA Drivers license; also must have been a licensed driver for at least one year (in any US state)Reliable vehicle with current registration and insuranceSelf-motivated and detail-oriented with ability to multi-task and ability to exercise sound judgment and independently analytical and communication skills with ability to build rapport with a team.
