Case Management Intensive Care Coordination Supervisor
Annual Salary: $57119
Work Schedule: Monday - Friday 8:30 a.m. - 5:00 p.m. with on-call responsibilities
The Case Management Supervisor (Intensive Care Coordination) provides day-to-day oversight of intensive care coordination and case management services for children adolescents adults and families impacted by serious emotional disturbance mental illness intellectual/developmental disabilities and substance use disorders. This position ensures services are timely compliant and clinically appropriate while strengthening staff performance and program results.
Key responsibilities include supervising approximately 68 Case Managers; guiding clinical assessments service planning utilization management and crisis support; coordinating hospital liaison and discharge planning; assigning and balancing team caseloads; training and onboarding staff; partnering with community stakeholders (including courts law enforcement and health providers); monitoring unit expenditures; and recommending program/policy improvements.
Required qualifications/skills:
- Bachelors degree in Human Services such as Social Work Psychology Sociology or Counseling.
- Three (3) years of experience providing case management services to the assigned client population.

Intensive Care Coordination (ICC) offers a higher level of support including clinical assessments and level-of-care determinations for individuals diagnosed with a Serious Emotional Disturbance (SED) or an Intellectual or Developmental Disability (IDD).
