Job Summary
Lyngblomsten is hiring a Caregiver Specialist to join The Gathering.The Caregiver Specialist provides support education and resources to family caregivers participating in our group respite program The Gathering. This role also fosters meaningful opportunities for caregivers to build connection community and social support.
The posted pay range of this hourly position is$25 to $ actual rate of pay offered within this range may depend on several factors such as FTE skills knowledge relevant education experience and market conditions. Additionally our organization values pay equity and considers the internal equity of our team when making any offer. Hiring at the maximum of the range is not typical.
Key Responsibilities
Conduct assessments for new participants interested in The Gathering coordinating directly with family caregivers.
Plan and facilitate opportunities for family caregivers to connect.
Provide ongoing education resources and support to family caregivers.
Support caregivers during transitions and address concerns related to safety or program eligibility.
Take opportunities to engage with The Gathering participants.
Collaborate as a team member and contribute to the growth of Lyngblomsten Community Services.
Represent the organization at outreach and professional networking events as needed.
Desired Skills and Experience
Bachelors degree in Social Work or Nursing; 3 years of experience supporting family caregivers preferred.
Strong understanding of dementia through professional and/or personal experience.
Experience facilitating caregiver support groups.
Minnesota Certified Caregiver Consultant preferred.
Strong assessment skills (physical mental social and family systems).
Excellent communication listening and time management skills.
Ability to manage multiple priorities and maintain professional boundaries.
Culturally responsive with experience working with diverse populations.
Proficient in Microsoft Office and virtual platforms (e.g. Zoom).
Reliable transportation; ability to work at both Como and Lino Lakes campuses.
